III. MOVEMENT OF BEDS
1. Mulberry Lodge, Sebastian County Retirement Center and Greenwood Retirement Center, are
requesting to transfer of twelve (12) beds from Mulberry Lodge RCF in Crawford County; six (6) beds to
Sebastian County Retirement Center; and six (6) beds to Greenwood Retirement Center both facilities are
located in Sebastian County.
2. Brooken Hill Health and Rehab is requesting an intercounty movement of beds. To move sixteen (16)
beds from the closed facility, Clarksville Nursing and Rehab, LLC in Johnson County; to move two (2) beds
from the closed facility, Deerview, LLC in Yell County; and to move an additional seven (7) beds from the
closed facility, Abbey Hill Nursing and Rehabilitation Center located in Garland County, a total of twentyfive (25) beds to their facility located in Sebastian County.
3. Rogers Health and Rehabilitation Center is requesting an intra-county transfer of twenty-three (23) beds
from The Green House Cottages of Northwest Arkansas, LLC to their facility. Both facilities are located in
Benton County.
